Care Coordinator – Addiction Treatment Clinic

About the role

TSB Poliklinikk, part of Sykehuset Telemark, is looking for a Care Coordinator (Forløpskoordinator) for its Skien team. This newly created position focuses on coordinating good patient pathways and integrated aftercare. The clinic serves young people and adults aged 16 and over with substance use and mental health problems, and works closely with the municipality, NAV and general practitioners.

Key responsibilities

• Coordinate patient pathways according to national guidelines and integrated aftercare, following national professional guidelines
• Act as each patient's fixed contact person throughout treatment, ensuring continuity and safety
• Plan and coordinate transitions between outpatient care, inpatient care and municipal services
• Take part in treatment meetings and follow up decisions and actions
• Advise patients and relatives about their rights and available offers
• Contribute to quality improvement and good cooperation with private partners
• Identify and follow up delays or bottlenecks in patient pathways

Required profile

• Bachelor degree in social studies or Norwegian authorisation as a nurse or nursing assistant
• Experience in intake coordination, patient pathways or rights assessment is an advantage
• Further education in mental health and addiction care is valued
• Fluent written and spoken Norwegian; driving licence category B
• Ability to work independently and in teams, with good communication skills
